Physical Forms for the Fall 2022 sports season are due to the Health Office no later than August 4th, 2022. Students will need to have an updated physical from their doctor and the forms completed in order to be allowed to try out for Soccer, Volleyball or Cross-Country. Physical forms are located in the Main Office, the Health Office and are linked at the bottom of this page. Please contact Mrs. Marmorato with questions.

The following sports are available to students in grade six through eight: co-ed soccer, girls volleyball, co-ed cross country, boys and girls basketball, girls softball and boys baseball.
Participation fee for sports is $50 per sport.
If you are interested in participating in a sport, please be sure to have a current physical form submitted along with the Coach's Authorization and Student-Athlete Emergency Information Form and the Student-Athlete and Parent/Guardian Acknowledgement Form.. A physical is good for 365 days. Once the physical form has been filled out by a physician and is turned in it will stay on file in the nurse's office for that term. If your physical is not current at the start of the season you will be unable to try-out or spectate at practice. Parents need to complete a Health History Update Questionnaire if their physical was completed more than 90 days prior to the first day of official practice. Parents should keep the concussion, cardiac emergencies, and eye injury fact sheets for your records.
